Sometimes, when resorption has excessively reduced the jawbone, it can be rebuiltthrough modern bone grafting techniques. Bone grafts can build up or fill in jawbone defects allowing the placement of dental implants. These techniques represent one of the greatest advances in modern dentistry.

DENTAL IMPLANTOLOGY
Dental implant placement is a type of oral surgical procedure during which titanium dental implants are inserted into the bone after the gum had been opened. There should be no pain when the dental implants are placed, in most cases local anaesthetic or IV sedation is used. Ususally patients can leave and go to work the next day, unless any surgical circumstances arises. Temporary denture is usually given for the healing period for aesthetic reasons. The healing time for implantation (if no other intervention is needed) is usually 3-4 months (except in the case of other dental implant systems which heal quicker) after which the healing screws are inserted. 2 weeks after the insertion, the abutments are fitted and mould is taken to plan the crowns or bridges. Within 10 working days the permanent crowns or bridges are placed onto the dental implants.
There are various types of restorations used on dental implants, which usually depends on the treatment objectives. These are removable prostheses (dental implants are used for retention), totally dental implant supported porcelain fused to metal or metal-free crowns or bridges, removable or fixed dental implant bridges, single or multiple crown(s) on dental implant(s).

* please note, the type of treatments performed at our London clinic are decided by the dentist. We only perform treatments in London that are less complex and implantology is only performed up to 3-4 implants. Crown and bridgeworks are only applied in London in certain cases (we mainly insert 3-4 crowns but only to implants not to own teeth).
GRAFTING WITH ARTIFICIAL BONE
Bone graft is a type of oral surgical procedure when bone is substituted with synthetic replacement materials instead of bone. These synthetic materials contains hydroxyapatite. These materials have many of the same bone forming properties as human bone and absolutely safe alternatives.

TYPES OF BONE BLOCKS
Bone block from the chin
Generally considered the best material for dental bone grafts, autogenous bone is the process whereby bone taken from adjacent sites of the patient such as the chin. Autogenous bone is the standard as it oftentimes has the most predictable results. It is considered the best type of dental bone grafting technique because the bone is live bone.
Artificial bone block
The procedure is the same as with the bone replacement of the chin except our specialists use only synthetic blocks to fill the areas of bone loss. Please note, this type of procedure usually has lower success rate then the one using bone from the the patient's body.

SINUS LIFT
Sinus lift is a great solution for those who has bone loss in the upper jaw area or implantation cannot take place because the sinuses are too close to the jaw. It usually advised to have for those patients who has not got enough bone in the upper jaw, when teeth are missing for a long time or the maxillary sinus is too close to the jaw for dental implants to be placed.
In these cases the aim is to increase bone quantity. During the procedure the doctor cuts the gum tissue near premolars or molars. A small, oval window is opened in the bone exposed. The sinus membrane is gently pushed up and away from the jaw.

Bone graft material is placed into the space, this way several millimeters of bone is added above the jaw. The incision is stitched and closed.

There are two possible treatment types, either performing the implantation together with sinus lift, in this case the healing time is 8 months or first the sinus lift is done and 6 months later the implantation will take place which again has to heal for another 4 months.

The type of treatment is decided by the specialist after personal consultation.

** Sinus lift is might be done separately. There are cases when sinus lift is done at session 1 - 6 months healing time then implantation at session 2 - 3-4 months healing time. Please note, with sinus lift it is not advised to fly for a week, earlier leave on own responsibility.
BONE GRAFT FROM HIP BONE
There are cases when implantologists use another part of the jaw for bone grafting (such as chin or back portions of the jaw, see above) but when there is not enough bone available, they have to turn to other parts of the body such as the hip bone (as the hip bone contains enough marrow, soft tissue within the bone).
The autograft is a procedure where the specialist collects bone from the pelvis above the hip joint. A cut is made (usually a couple of inches long) and the required amount of bone is taken out. According to the amount of bone needed a small plastic tube may be placed into the wound before stitched.
After the operation you are likely to feel some pain for at least a few days even if only a small amount of bone has been collected. In the case of a larger amount of bone collecting the area might hurt for even a couple of weeks. Dressing is put over the wound after surgery, this should be worn until the stitches have been removed. You should expect to stay in hospital for a couple of days but it depends on the treatment type involved at the same time.
